You will only need one or the other of the letters below:
(This is for instructors entering the instructor processes for the first time or are at the provisional level from the grandfathering process.)
(This is for Instructors who entered the program during the previous grandfathering process and are identified as a “Grandfathered Instructor”)
Please compile the required information (Letter from your agency, certificates, letters of recommendation specified by the policy, Task Book completed to the correct level, etc). Mail this to:
